United States of America: Caseworker, Intensive Services

The Caseworker ensures efficient delivery of quality medical and intensive/extended case management services to clients who face significant barriers, including physical disabilities or chronic/acute medical issues. S/he supports cases through assessment, planning, service procurement, delivery, coordination and monitoring. The Caseworker for Intensive Services will ensure all services are offered in an inclusive, respectful and professional manner. This position is a regular, full-time position.

Major Responsibilities:

  • Provide intensive case management services to clients with special needs: physical disabilities, infectious diseases or clients who have chronic or acute medical issues that contribute to self-sufficiency barriers.
  • Conduct intake, assessments, development of ICM service plans, goals matrices, ensure at least one monthly contact with all ICM clients, home visits, medical adherence and medical appointments.
  • Coordinate with the Medical Services Coordinator and Behavioral Health and Wellbeing Promotion Supervisor to provide client-centered services.
  • Provide services to clients who experience set-backs or other challenges that impede their path to self-sufficiency.
  • Provide transportation for clients to appointments.
  • Ensure quality and complete documentation of all ICM files as per PC Guidelines.
  • Develop professional relationships with clients and maintain ongoing connection with clients and families.
  • Develop and maintain relations with medical and social services providers to better serve ICM clients.
  • Assure client awareness of all IRC programs and available community services; advocate for accessibility for all services.
  • Provision of client care with medical, mental health, and other providers, and provide linkages to agency, community and government services and monitoring referral completion including state insurance continuation programs, medication assistance programs, and benefits/health coverage programs such as SSI, Medicaid, and housing programs.
  • Complete documentation in case management database system in a timely manner.
  • Demonstrate respect and sensitivity for cultural differences.
  • Positively represent agency in community meetings.
  • Other related duties as assigned.

Qualifications

  • Undergraduate degree in social services or related fields or comparable experience may be substituted.
  • At least 2 years case management experience, preferably with refugees in the Medical /Behavioral Health field.
  • Demonstrated understanding of fiscal management and reporting; Excellent verbal and written communication skills and the ability to communicate with limited-English speakers in a multi-cultural environment; Experience working in a multi-cultural environment; Advanced computer skills including word processing, Excel, database systems, desktop publishing, and PowerPoint.
  • Bi-lingual ability desirable.
  • Valid driver’s license; current insurance and reliable transportation.

Work Environment

  • Both standard office work environment and ‘field’ environment in and around the client service delivery area.
