The Health Liaison is responsible for programming that ensures the mental and medical health needs of clients are promptly identified and treated, that they are able to establish a medical home for ongoing care, and that they have access to educational resources to support the health and wellness of themselves and their families.
Major Responsibilities:
Responsibilities include, but are not limited to:
Client Services:
- Provide assistance to refugees to ensure timely provision and coordination of medical services, particularly for those with serious health conditions.
- Facilitate communication between clients, local health department, medical providers, and resettlement agency staff to support provision of needed services for newly arrived refugees and those with serious health conditions.
- Oversee delivery of health related orientation information to newly arrived refugees as part of local cultural orientation program and provide written information on health topics to refugees as needed.
- Arrange for transportation as needed for refugees to attend follow-up medical appointments.
- Provide information to refugees on medical insurance plans including referrals to the health insurance marketplace as appropriate.
Provider Relations:
- Provide training for community health service providers on the special cultural and linguistic needs of refugees.
- Develop and maintain up-to-date list of community health resources; conduct outreach to potential new providers.
Contract Compliance and Documentation:
- Ensure compliance with both case file management and reporting requirements. This entails regular and timely maintenance of both hard-copy and electronic files per best practice standards.
- Coordinate with State Refugee Health Coordinator on any unusual or difficult cases that occur or that may benefit from support from the Virginia Department of Health.
- Participate in monthly conference call facilitated by State Refugee Health Coordinator and report health data to the Office of Newcomer Services as required.
Oversight of Community Health Promotion Program:
- Provide oversight to the Community Health Promotion Program including the development and delivery of staff training and quality assurance techniques to support in-home and community based healthcare navigation and education.
- Provide ongoing supervision of the Community Health Program Coordinator, ensuring that program goals are being met.
- Assist Site Manager and Community Health Program Coordinator to prepare proposals for future funding as well as reports to current funders.
Team/Office Participation:
- Participate in all program meetings, staff development activities, and other duties as assigned.
- Comply with all policies, procedures and protocols of the agency, placing importance on client confidentiality.

Qualifications
- Bachelor’s degree in social work, public health, or a related field of study, or equivalent work experience.
- One to two years relevant work experience in human services field.
- Demonstrated success working and communicating effectively in a multi-cultural environment.
- Prior supervisory experience preferred.
- Proven ability to contribute both independently and as a key team member.
- Self- starter with excellent problem solving skills combined with the proven ability to multi-task, prioritize duties, and manage time effectively.
- Fluent in English, both spoken and written; bilingual ability in a language spoken by the local refugee population is desired.
- Proficient in Microsoft Office applications (Word, Excel, Outlook).
- Valid driver’s license, access to a reliable vehicle with current insurance.
Working Environment:
- A combination of standard office environment and ‘field’ time in and around the service delivery area to perform the above outlined responsibilities.
- May require occasional weekend and/or evening work.
